What happened
Injured right shoulder by slamming into the concrete door frame of a patient procedure room. My shoulder hurt for the [REDACTED] but figured that it would get better. Pain increased each [REDACTED] and by Fri[REDACTED] night it was extreme with very limited mobility. [REDACTED] [REDACTED] morning I called Urgent Care and spoke with a Triage nurse and was directed to the Emergency Room. I was seen by a physician medicated and x-rayed and consult was put in for Orthopedics. I notified my supervisor by phone on [REDACTED] [REDACTED]. I filled out this same form on [REDACTED] [REDACTED]. I saw an orthopedic doctor on [REDACTED] and was given work restriction documentation and sent that.
