EE LEFT [REDACTED] STATING HE WAS STRESSED OUT DUE TO WORK. UPON REVIEWING HIS OPTIONS HR RECOMMENDED THAT EE SHOULD SEEK TREATMENT THROUGH WORK COMP. I CALLED EE TO FOLLOW UP WITH HIS REQUEST FROM [REDACTED] ON HOW TO GO ABOUT [REDACTED]. EE WAS ADVISED THAT HE CAN GO TO A NEARBY SISTER STORE TO FILL OUT THE REPORT TO SEEK A CLAIM THROUGH WORKERS COMP. EE STATED THAT HE WILL BE LOOKING INTO HIS OPTIONS. Report will be updated once we find out more information to see if he will be seeking treatment. EE stated that he would prefer to go to store #59 if he were to fill out a report. Report filled out at store #59 [REDACTED]: On the phone with HR when started yelling and getting aggressive. I started feeling nauseas sweaty anxious shaken. Didn t know he could go to Concentra because he was suspended and did not know.
