Injured employee was climbing an adjustable ladder The ladder collapsed and the employee fell to the concrete floor His ankle wedged in the ladder twisting it as he fell He also struck his head on the concrete floor and scraped his right wrist Employee may have briefly lost consciousness Employee suffered abrasions and bruising to the back of his head and abrasions on the wrist He was experiencing severe pain in his ankle and was transported by company vehicle to urgent care for evaluation After further evaluation it was determined that injured employee incurred a hairline fracture in his tibia
