Starting the paper machine up pulling the rope to lower the festoon to rethread paper through the machine.
What happened
The Operator tried to start up the line and the paper broke. He pulled on the rope with excessive force to lower the festoon rollers to thread the paper through causing the metal counterweight to exit the top of the frame and fall towards the ground striking him above the right eye.
