Before the incident
The employee was pushing the end of a 6 plastic pipe.
What happened
The pipe would not fit into the frame of the mold. The employee was trying to force the pipe into the area by pushing on the end. A second employee put a strap on the pipe between the effected employee 's hands and started pulling on the strap. The strap slipped and caught the effected employee 's pinky and ring finger between the edge of the pipe and strap.

The employees left hand pinky and ring finger were cut.
