Employee said it wasn 't one particular incident or action to cause this but maybe a result over time on the job. Employee finished up a few more accounts without lifting or exerting himself before deciding to go to the ER (Garden [REDACTED] ER in [REDACTED]). Employee never reported it and sent an email later in the evening that he would be out the [REDACTED] but with no other details. Employee stated the ER doctor diagnosed him with [REDACTED] and signed him out for 2 full days returning to work on [REDACTED]. Employee went on a follow up with his own PCP on [REDACTED] who confirmed it was [REDACTED]. Employee did not officially complete his day leaving several accounts to be seen and did not report to district manager.
