Before the incident
EE stated that he was pushing a u-boat filled with product from the outside of the store to the inside of the store. He stated that Walgreens had just gotten their own delivery and there was no room in the back to work the product. As the EE was pushing the u-boat he stated that he felt a pop in his shoulder and his groin.
What happened
On [REDACTED] from [REDACTED] EE was making a delivery to Walgreens #12869 located at [REDACTED]. During that time the EE stated that he was pushing a u-boat filled with product from the outside of the store to the inside of the store. He stated that Walgreens had just gotten their own delivery and there was no room in the back to work the product. As the EE was pushing the u-boat he stated that he felt a pop in his shoulder and his groin. EE continued and finished his day. He never mentioned any issues to his supervisors. EE worked and completed his route on [REDACTED] and then went to get a new DOT med card. The EE was told by the physicians assistant that he had a hernia in his groin area yet was still issued a two year health card. EE brought the health card to his supervisor on [REDACTED] and then proceeded to tell his supervisor that he injured himself on [REDACTED] and wanted to get checked out by Concentra. EE filled out his employee injury statement and is seeking treatment.
