Employee allegedly slipped and fell while cleaning in the sludge pit room. Employee did not report injury to supervisor or complete incident report when incident occurred. Sludge pit crane found to be blocking camera angle no footage of incident. Employee contacted HR later in afternoon ([REDACTED]) to report injury to management staff and was directed to Concentra for treatment by EHS. On-going investigation.
