What happened
Employee 's press was down for a mold change. Employee began mopping up floor moved out of the way for another employee and slipped fell on her bottom. Employee was cleaning behind press 3 outside of view of security cameras. Employee refused medical treatment after incident occurred stating a sore painful bottom. Incident occurred on [REDACTED] [REDACTED] and employee was off the entire following week due to layoffs. Employee then returned on [REDACTED] [REDACTED] and provided medical paperwork to her supervisor. Employee was sent home and her temp agency was contacted for investigation. Agency terminated employee due to failed DT.
