[REDACTED] was unloading door 33 with [REDACTED]. The driver had put his pallet jack on top of some beams on the back of the truck. It was clipped to the trailer wall. [REDACTED] picked up the pallet jack with his lift and when [REDACTED] attempted to unhook the pallet jack from the wall the handle swung around and hit him in the face.
