At [REDACTED] an employee was carrying a bundle of shingles to the area he needed for the roof. While carrying the shingles he tripped on a step his body fell forward at the same time he swung his arms out grabbed the scaffolding railing to catch himself hitting [REDACTED] in the back of the head right below where our MSA hard hats protect. Immediately after hitting [REDACTED] the employee dropped the shingles onto the roof asked [REDACTED] if he was okay to which [REDACTED] did not respond. The team lead in that department called [REDACTED] over. [REDACTED] gave him an ice pack and then a few [REDACTED] later went to check on [REDACTED] again [REDACTED] told him his dizziness was getting worse Rollen one of our area coordinators was called over to which [REDACTED] came and told [REDACTED] and I what was happening and at [REDACTED] [REDACTED] was took to the hospital. At [REDACTED] a CT scan was completed at [REDACTED] the nurse told me it was definitely a concussion I left due to [RED
