Before the incident
PCA [REDACTED] was trying to help a pt that was yelling out kicking and trying to get out of bed. Nurse was getting medications ready to give ([REDACTED]) to pt to help calm pt down. Daughter was at bedside and [REDACTED] on other bedside and pt grab a hold of [REDACTED] 's right wrist and twisted and she yelled out in pain and started to cry. This nurse [REDACTED] went into help assist and relieve [REDACTED]. [REDACTED] is complaining of wrist pain and ice was applied. [REDACTED] also states he tried to pull on her pants and bring them down. Pt was kicking towards daughter and towards PCA.
