Before the incident
Set-Up Tech was working toward starting up the IMM (E0) trying to remove plastic piece that was stuck.
What happened
Setup Tech was starting up press E0 and after the first cycle plastic was stuck in the mold. As he was removing the stuck plastic he unknowingly pushed the slide in. The slide is a mechanical part of the mold that helps to form the parts. He tried restarting the press but with the slide mislocated it caused the horn pin to break. The horn pin is the guide that moves the slide in and out. After realizing what happened he opened the modl back up tto correct the potion of the slide. The slide has two brass water pipes with water hoses connected to thme. As he was moving the slide to the outward position by hand he also disconnected one fo the water hoses which was carrying 180 degree water. In a panic he thgout he could quickly reconnect the water hose with the water coming out of it. He ended up with burns on his left wrist and below his left armpit down to the side his waist area.

While starting up IMM Setup Tech started to remove plastic that was stuck in mold on the first shot. When doing this he was unaware that he pushed the slide that ended up break the horn pin after starting. Setup Tech tried pulling the slide back into position by using the water hoses as a grip which caused the hot and cold hoses to detach from the brass pipe spraying hot water on the Setup Tech. Received 2nd degree burn to left forearm & 1st degree burn of chest wall
