Before the incident
Employee was at freezer and while walking through roll up door on new freezer side when she stepped into the freezer and employee slipped backwards on a patch of black ice that built up from condensation. Employee stated she hurt her back and head when she fell. no head injury was observed on video footage. Written statement from employee states possible whip lash.
