Before the incident
Employee states he looked for the stair access to the west but did not see the stairs to access the concrete pad. He then decided to access the concrete pad via a piece of plywood that was placed against the concrete pad that is raised 24 inches in height.
What happened
The piece of plywood then gave way causing employee to slip and his left shin to make contact with the edge of the concrete slab.
