Before the incident
Loading the boxformer. EE removed the top sheet of cardboard from a pallet of boxes and dropped it into the cardboard hopper.
What happened
EE dropped off cardboard into the hopper. When EE turned to go back to the boxformer their left foot hit the frame of the pallet lift. EE tripped and fell onto knees and hands.
