SCRUB NURSE TOOK THE NEEDLE HOLDER AND TURNED TO PLACE THE SUTURE IN THE NEEDLE BOX. NEEDLE HOLDER SLIPPED AND THE SUTURE WHEN INTO THE SCRUB NURSE 'S INDEX FINGER.
