DURING SURGERY THE ASSISTANT FINISHED SUTURING ONE OF THE PATIENT 'S INCICISIONS WHILE THE SURGEON WAS OPERATING ON ANOTHER INCISION. THE ASSISTANT HANDED THE SCRUB THE SUTURE NEEDLE HOLDER. SCRUB NURSE TOOK THE NEEDLE HOLDER AND TURNED TO PLACE THE SUTURE IN THE NEEDLE BOX. NEEDLE HOLDER SLIPPED AND THE SUTURE WHEN INTO THE SCRUB NURSE 'S INDEX FINGER.
