Before the incident
Operating order Picker performing the S3 pick function.
What happened
Team member was operating their Order Picker performing the [REDACTED] pick function. They proceeded into aisle BL. They entered the aisle from the [REDACTED] side of the aisle and was proceeding in the direction of the [REDACTED]. As they were traveling in this direction the left-hand guide wheel ended up catching onto the open end of a separation in the guiderail that (roughly around bays 013-014 even side) with the guiderail being separated and pushed in. With the guide wheel colliding with the lip of the exposed guiderail this resulted in the Order Picker coming to a stop and shifting his picker in a diagonal position with the front of the OP positioned against guide rail(even side) and the forks (holding a pallet and hamper) shifted to the odd side of the aisle with the forks and pallet ending up in the A and B levels of the racking. When their picker made contact and his forward motion stopped resulting with his face colliding with the metal grate on the front of the picker. The
