Before the incident
[REDACTED] was lowering height of a stretcher to wheelchair height.
What happened
at approximately [REDACTED] I was contacted by ee#1 while moving a patient he injured his back. ee #1 stated that he was using the [REDACTED] he and ee #2 were lowering the cot from load height to wheelchair height when due to lowering at an angle the locks didn 't catch and caused it to go lower than expected and therefore caused strain to the lower back of ee #1. The patient was then safely assisted to a wheelchair and no injuries report from ee #2 or the patient. AIR report was obtained and ee #1 was referred to ProTeam... summary cuts off here
