What happened
Work started last night at salmon creek pulling up carpet and putting down adhesive subflooring in employee entrance area and waiting room. We were told that that subfloor was safe to walk on. [REDACTED] came in the employee and entrance and her shoes got stuck . Her hands were full and in trying to move wrenched her left ankle and left buttock. An employee from our janitorial service witnessed the event and came back into the building to get [REDACTED] unstuck and into an area where the floors were not affected. [REDACTED] is claiming she is in too much pain to work and is going to Zoom Care [REDACTED].
