Before the incident
Removing a fuse from electrical panel
What happened
The maintenance employee was attempting to change a fuse at the main plant electrical panel. The employee started to take off the 2nd bolt attached to the fuse mounting bracket. As the the employee was turning the ratchet he made contact with the disconnect handle on the electrical panel there was a small explosion associated with a very bright flash.
