What happened
Employee was steaming a phenol leak at the phenol pump in the tank farm when he felt a burning sensation on his left leg. A small tear on the left leg of his chemical suit caused by a snag on a sharp edge of insulation cladding on the pump inlet allowed steam and phenol to enter his suit and contact his leg.
