Before the incident
IW was stationed behind a window rigged to a drop pulley with a rope that included a metal protrusion on the top of the window frame. IW 's performance included positioning head through the window to achieve a scare effect for guests.
What happened
When the window was dropped using the pulley the knotted rope caught on the widow hitch to the side of the opening causing the window to stick up more than usual designed. When the IW moved his head through the opening his lower lip and teeth caught on the metal protrusion.
