Before the incident
The horizontal pan adjustment bar (usually adjusted by a wheel) was not staying in place and slowly moving inwards as the pans entered the pan stacker on the bun line This caused the pans to not go in straight resulting in jams The employee had to keep pulling pans back from the infeed conveyor due to this and while doing so another pan came in behind him and hit into his upper left arm causing a burn
