Employee was reassembling servo actuator assembly on a groove cutting machine. Machine was locked out according to LOTO procedure.

When they lowered the suspended spindle assembly onto the repaired servo actuator assembly by releasing the strap they had the tip of their left ring finger between the piece that was being lowered and the nut on the servo actuator assembly causing finger to become pinched between spindle assembly and nut.

Open fracture to the tip of the left ring finger.
