Three maintenance employees were putting belts back on the heat mixer. Heat mixer was locked out. Two of the employees were on the level under the blender installing the belts on the pulleys (16 belts). They had another employee at the mixer level holding the belts in place that had already been installed while the two employees began to install the next belt.

As they rolled the next belt on the employees hand was pinched between the pulley and the frame of the mixer.

Carpal avulsion fracture to left hand.
