The maintenance team in the General Shop was in the process of relocating a 20foot agitator shaft into the building for a rebuild Due to the width of the shaft exceeding that of the rollup door the team needed to reposition the shaft during its entry into the building During the relocation as the forklift was descending the ramp into the shop the load was jostled causing the front strap to slip from its rigging attachment point and make contact with the teammates foot that needed offsite medical treatment
