We were attempting to get the 4 spout unplugged and at times the operators used a sledgehammer to strike a rod to break through While one operator was holding the rod the 2nd operator swung the hammer hitting rod and the hammer head came off the handle on the down swing of the next attempt striking the operator holding the rod in the side of the ankle I have had all the areas inspect their sledgehammers and will not use them until further notice
