Before the incident
Team member was cleaning the slitter from the the slitter platform While cleaning he attempted to reach for an air gun hanging on the cage of the machine for assistance While reaching for the air gun the team member took a step with his right foot onto the top of the ladder that is used to access the slitter while keeping his left foot planted on the slitter platform His right foot slipped off the ladder (6ft) causing his body to fall on the ground while his left foot was caught on the slitter platform His left leg being caught caused his left leg to be pulled by his body weight causing injury in the left knee area
