Around [REDACTED] on [REDACTED] maintenance technicians were extracting a broken part of a lateral adjustment wheel on the print section of the rotary die cutter The wheel had broken off while using an extractor so other measures were taken to remove the remaining part A new hazard was now exposed as the broken part was now revealing a sharp end A socket wrench was now being used to remove the remaining broken part As the technician was undoing the bolt he lost his balance fell forward and suffered a laceration to his right thumb and right forearm Paper towels were quickly applied to stop the bleeding The maintenance supervisor notified the site safety manager and a decision was made to transport the technician to the urgent care
