What happened
[REDACTED] explained that she slipped on the water next to an ice chest with refreshments and fell on her right knee. [REDACTED] explained that she saw a wet floor sign on the opposite end away from where the ice chest was. [REDACTED] then provided [REDACTED] with the employee at work injury forms and after examining her right knee [REDACTED] investigated the ice chest area with water on the floor and took photos which are attached to this report. [REDACTED] then decided to go to the emergency room to have x-rays taken of her knee. No breaks were found in the x-ray just bruising and swollen.
