Before the incident
Employee was making a delivery for one of our customers The customers we were delivering to is a storage units facility when going past the gate there is 6 feet of [REDACTED] that is uneven and have plenty of pot holes As the driver was pulling the pallet jack he was constantly turning his back switching from walking backwards and forwards the driver was doing this to make sure the pallet wasnt going to tip over at any point as the driver was turning to face forward he did not see one of the pot holes and stepped inside it twisting his foot backwards The driver called the operations manager and then called the safety associate the employee states that the pain level is at a 6 or 7 when he walks bur has 0 pain when he sits and drives
