Before the incident
At [REDACTED] we had an accident with an employee. She stated she went on break and was in the womens restroom where there was a small puddle of water. We did not notice any leaks in the area but did notice a bunch of open water bottles around the area. She then slipped and fell hitting her shoulder neck against the lockers in an awkward position. She claims to have pain in her neck shoulder back and knee. She struggled to get up and we asked her if she felt like she couldnt move in which she said she didnt need an ambulance and just needed a few minutes. She did seek medical.
