Grabbing a headlight in the cab of the truck from the overhead compartment. This was on the passenger side of the truck. In [REDACTED].

Reached for the headlight and when he placed foot down missed steps. He fell back and landed on the asphalt. Back was sore after the incident and stated the wind was knocked out of him. On [REDACTED] had hard time breathing and pain in ribs while unloading a truck.

Sore middle back initially. Later developed rib and breathing issues after unloading trailer of product in MN. Went to the ER in MN.

Missed steps in truck cab and fell on asphalt while exiting.
