Before the incident

Investigationg why conveyor belt was slipping and throwing a lime substance on the underside of the belt to create friction. Employee reached into a guarded area to try to dislodge some sugar that had accumulate on the belt at the roller

What happened

When employee reached into the area between the belt and the roller his elbow was caught in the pinch point and his arm was pulled into the conveyor between belt and roller
