Stringing up pay off and was attempting to lower pay off arm
What happened
Team member was attempting to lower payoff arm 4A when the arm itself got hung up on top of another payoff on the opposite side. Team member took his finger off the payoff drop button and went to fix the hang up. When they moved the opposite side payoff arm payoff arm 4A fell with the full weight of the arm and struck team member in the forehead causing a roughly 3 laceration on the top half of forehead directly over left eye.
