Employee was air testing a section of pipe. He had his face in front of the cap when he went to remove it instead of to the side or rotating pipe end down towards the floor. He had forgotten to bleed of the air prior to removing the cap and the cap flew off hitting him in the face and mouth. This knocked him back down causing back of head to hit something.
