[REDACTED] reported that this happened to him on [REDACTED] [REDACTED]. I asked [REDACTED] if he needed medical help and he declined. I explained the process of going to our work comp clinic and if he chose to get medical care now or in the coming days he needed to let me or another manager know. He was asked several times if he wanted to go seek medical treatment and he continually declined. [REDACTED] came in on [REDACTED] and gave me a medical status report from Rockhill Othopaedic Specialists. He said they told him he had 3 lacerations contusions on the top and bottom of his foot stating that he had a fracture in the bottom of his foot. I told [REDACTED] that he would need to start with Concentra on [REDACTED]. Right foot injury.
