Before the incident

Signaling to the forklift operator to bring the beam into position. He then had the operator stop because the beam was wedged between the concrete wall and the column.

What happened

An iron beam was being set in place ready to be bolted up. While secured to the forklift the beam got bound up between the connection point and the concrete wall. The beam broke loose from the securing bridge clamp and fell approximately 3 feet down onto the handrail of the boomlift. The team member 's hand was in the line of fire and the beam smashed the end of their thumb
