What happened
Employee was taking out boxes to the outside dumpster using a manual pallet jack. The ground is on a declining hill. EE was going down the hill and the pallet jack started to accelerate too fast. The wheels of the jack smashed EE foot and caused EE to lose EE balance. EE then fell over the jack which cause EE to land hard on back. EE is having pain and swelling in right foot. EE is also having pain in lower back area.
