Running plywood through double end tennenor machine

[REDACTED] sustained an injury while operating the double end tenon machine injuring his left middle finger. He claims to have turned off the machine before cleaning it. While removing wood debris from the blade area [REDACTED] inserted his hand to clean the saw blade area which he believed was off but the saw blade continued spinning unaware that it was still in motion.
