Before the incident
Employee had climbed the stairwell to the bin conveyor deck. In turning the corner to walk across the deck a section of the grating had been removed by contractors and the employee stepped in the hole striking left shin against the grating.
What happened
Contractors had removed floor grating on the conveyor deck and due to lightening were ordered to seek shelter before covering hole or placement of danger tape. Employee ascended stairwell to investigate conveyor motor sending error code to computer screen. Upon reaching conveyor deck level employee turned corner and stepped into hol striking left shin against existing grating and flooring frame.
