Root Causes1 Improper body placement. Associate used steering wheel as one part of a 3point contact. Never squared to the steps while exiting cab.2 Inadequate work procedure. There are grab rails available on each side of the door but SPP 33 does not specify what to use for the 3 points of contact.3 Physical impairment. Associate previously stated issues with knee replacements back spasms and other major physical issues.
