Before the incident
Employee was dismounting a tire sized 275 60 20. He was using the proper tire bar and dismounting coats machinery.
What happened
Employee dismounted the front of the tire with no issues. He inserted the tire bar into the gap per standard procedure pressed down on the left side of the tire and leaned over the tire. When leaning over his hand slipped off the tire allowing the bar to swing back up and hit him above on the eyebrow.
