Before the incident
[REDACTED] was exiting forklift and slipped while descending steps. His feet slipped out from under him.
What happened
As he fell he braced himself with his left hand while holding on to the handrail with his right hand. His left hand struck the top of the DEF tank causing a dislocated left middle finger. Crewmember exited the lift facing outward and not facing the equipment as he had been trained.
