On [REDACTED] at approximately [REDACTED] employee stated he was performing a 425 commercial turn on at [REDACTED]. The employee started the turn on and was removing a 1 1 4-inch cap from the outlet side of the meter set when he states I felt pop crack in my lower back. When the employee arrived at home he reported I started feeling more and more pain and was unable to sleep last night. The employee gave notification to the supervisor at [REDACTED] today via text message that he was taking a sick day today. The employee later notified the supervisor via text message at [REDACTED] that he was heading to his personal doctor because he believed he may have sustained an injury in his lower back yesterday at work. The employee finally called his supervisor at [REDACTED] and informed his supervisor that his doctor informed him he needed to contact his place of employment if he believes he sustained an injury at work.
