Before the incident
The employee was working on a gas service retirement. The employee and his crew completed the service disconnection at the gas main. Once a service is disconnected from the gas main the service riser is removed from the building. The gate was locked so they used a ladder on either side of the fence to gain access to the vacant property to remove the service riser.

Employee was using two ladders to climb over a fence. He had a ladder on either side and while coming down the other side he slipped off the ladder.
