What happened
Employee was under dock plate finishing the detail with Production Maintenance (PM) on the forklift using the forks to support the dock plate in an upright position so the Employee could finish his detail under the dock plate. While employee was working the PM backed out from supporting the dock plate and the front of the dock plate struck the employee on the right rear shoulder.
