Before the incident
Associate was on Line 5 Upsetter at [REDACTED]. There was a robot #4 gripper fault. Associate locked out at main lockout point for the robots and entered the cell. Gripper pads needed to be changed on robot #4. Associate did not lockout air valves for borth robots.
What happened
Associate attempted to change the gripper pads with his right hand. After the gripper pad was removed from both grippers the springs emerged closing the grippers on their right hand.
