Before the incident
Operator was trying to move the heading slide into the end of kick to be able to set kick-outs.
What happened
As the operator tried to jog the heading slide in the punch due to the punch being in the wrong station it exploded when the punch came in contact with the die which caused shrapnel to project hitting the chin and truck of the body.
